Placement Support and Outreach

Flexible support for young people and families

Kanda’s placement support and outreach programs provide flexible, trauma-informed support services for families and young people experiencing significant challenges in their lives.

By offering practical and emotional support, we help young people maintain stable and enduring out-of-home placements. Our goal is to help young people strengthen relationships, build resilience and achieve positive long-term outcomes.

Supporting stable and connected placements

Stable placements grow from trust, understanding and consistency.

Kanda’s Queensland placement support services are designed to help families, carers and care teams maintain strong, positive connections with the children and young people in their care.

We provide:

  • Therapeutic and practical guidance to stabilise placements and strengthen care environments
  • Collaboration with families, foster and kinship carers, and departmental partners
  • Family time and transport coordination to maintain meaningful contact and connection
  • Support for carers through coaching, problem-solving and trauma-informed strategies

Kanda is an approved provider of interstate programs such as the Casework Support Scheme (CSS). This enables us to tailor support packages to ensure continuity of care for children and families relocating from NSW to Queensland.

Family Support Services and Outreach

Our Family Support and Outreach programs are designed to support families and communities through tailored, evidence-informed initiatives that address their unique challenges. By maintaining flexible, responsive services, we aim to minimise disruptions and increase the likelihood of stable, long-term placements and positive outcomes for children and their families.

Services we offer:

Supervised Family Contact

We provide safe, welcoming spaces and experienced supervision to help families reconnect and rebuild trust. Our trained staff facilitate constructive conversations, boundary-setting, and relationship-building activities to support healthier interactions and improved connections.

In home support

Our in-home supports offer practical strategies and emotional guidance to address day-to-day challenges. We work with families to develop customised tools, routines, and communication approaches that promote stability, resilience, and a supportive home environment.

Education support

Our team collaborate with schools to remove educational barriers. This includes advocacy with educators, behaviour management and emotional support and coordinating resources to help children achieve their full potential.

Transport services

We provide reliable, tailored transport solutions to support consistent school attendance, meaningful family contact, and participation in voluntary activities. Our transport services are designed to reduce barriers and increase opportunities for engagement.

Assertive outreach

For families and individuals facing significant challenges, we offer proactive, intensive support designed to stabilise situations, connect them with essential resources, and work toward sustainable improvements. This approach emphasises timely intervention, ongoing assessment, and collaborative goal setting.

Our trauma-informed approach

All staff are trained in the Sanctuary Model, Therapeutic Crisis Intervention (TCI), and Hope and Healing Framework. These trauma-informed approaches focus on healing, emotional safety, and positive relationships.

By learning how to cope with stress and prevent trauma symptoms, we can better manage conflict and help our clients form healthy relationships.

Placement support FAQs

Placement support refers to targeted help for families, carers, and care teams to help maintain stable, nurturing environments for children and young people in care.

Placement support is available to children and young people in the Queensland child protection system, along with their carers, families, and support networks. Referrals are generally made through the Department of Family and Community Services or related partner agencies.

Outreach and social support services for families provide tailored support plans and community-based assistance to strengthen family capacity and prevent crises. These programs offer practical help, guidance, and access to resources that address challenges such as financial stress, housing instability, or parenting difficulties.

Placement support focuses on stability within existing care arrangements, while outreach extends into the community to prevent escalation and promote family wellbeing. Together, these programs ensure continuity of care by supporting children, young people, and families across all stages of need.

Kanda works closely with the Queensland Department of Family and Community Services or related partner agencies, housing and education services, and local community organisations to coordinate holistic, evidence-based responses. Our model promotes shared accountability, clear communication, and seamless transitions between services.

Make a Referral

Referrals are generally made through the Department of Family and Community Services or related partner agencies. When we receive a referral, we thoroughly review the child and young person’s background, care needs, and any risks to ensure we can provide the right support.
